This is the Consumer Health Data Privacy Policy of The Civic Advocate. It is published under chapter 19.373 RCW (Washington's My Health My Data Act) and Nevada's consumer health data law.
Consumer health data we collect, and why
We collect these categories of consumer health data, only from what you choose to give us:
- Your health conditions and diagnoses.
- How your conditions affect your daily life and your ability to work.
- Your treatments, medications, and side effects, and the names of the health care providers who treat you.
- Anything else about your health that you tell us by form, chat, email, phone, or text message.
Why we collect it and how we use it. We use this information only to (1) decide whether we can help with your Social Security disability claim; (2) if you hire us, prepare and present your claim to the Social Security Administration; (3) respond to you; and (4) keep records the law requires. We do not use it for advertising, we do not sell it, and we will not use it for any other purpose without first telling you and asking for your consent.
Where we collect it from
- You, through our form, chat, email, phone calls, and text messages.
- A person you authorize to complete the form for you.
- After you hire us, and only with your authorization: your health care providers, your prior representative, the Social Security Administration, and state disability agencies.
Consumer health data we share
Before you hire us, we do not share your consumer health data with any third party. We store it with the service provider named below, which processes it only on our behalf. After you hire us, we share the categories listed above with the parties below, as needed to handle your claim and only as you authorize.
